Appointments and Referrals for Patients
If you are looking for an appointment at Allegheny Brain and Spine Surgeons, we ask that a doctor refer you to our practice. We will need you to fill out the attached form
and include reports of your diagnostic imaging that you may have completed. Our medical staff will review your records and give you or your referring physician a call with an appointment. Attach the form and fax it to 814-946-1397.

For a New Patient Appointment You Will Need
- Driver's license
- Insurance card / Workers' Compensation Claim and contact information
- Fill out a New Patient Questionnaire and attach the document
- A copy of all diagnostic studies (i.e. MRI, X-rays, CT, myelogram) - Please bring the CD
- Copies of previous treatments for the condition you are seeing the doctor
For Established Patients You Will Need
- Insurance card / Workers' Compensation Claim and contact information
- A copy of all diagnostic studies (i.e. MRI, X-rays, CT, myelogram) - Please bring the CD
